REQUIREMENTS FOR INTERVIEW

 
1. 100 Point Identification Checklist

The items listed below in the table provide details of the types of identification and the points that are ascribed to each identification category. All employees of Colbrow Healthcare must be able to supply at least 100 points of identification material to validate who they are.


70 points

  • Current Australian or birthplace passport
  • Birth Certificate
  • Birth Card issued by Australian State Department of Births
  • Citizenship Certificate
  • Visa documentation with identification papers


40 points

 

  • Tertiary student identification with photo identification
  • Australian State License (with ID) - motor vehicle/ motor cycle
  • Centrelink or Pension ID card
  • Public Employee identification card with photo identification



35 points
 

  • Mortgage Document
  • Council rates notice for current address
  • Document for current employer (within 2 years of previous employer)                 
  • Land Titles Office record
  • Document from Credit Reference association of Australia (CRAA)

 

25 points 

 

  • NBV Registration Certificate
  • Educational outcomes records for last ten years
  • Current rent receipt from a licensed real estate agent
  • Lease/Rent agreement                                                                                   
  • Electoral Roll card from Australian Electoral Commission
  • Medicare Card
  • Foreign Driver’s license (with photo ID)
  • Current Telephone, electricity or gas utility invoice
  • Current credit card or Account card from a bank, building society or credit union

 

2. Other Important Documents

  1. Tax file number
  2. Banking details
  3. NBV Registration card and Nursing Degree Certificate
  4. Certificates (Post Grad/Seminars/First Aid/etc…)
  5. Passport Photo
  6. A copy of any immunisation records
  7. Current Police Check
  8. Working with Children Check (WWCC)
  9. Vaccination Details

 

Note:

It’s a requirement of Colbrow Healthcare that you have a current police check that has been done within the last 12 months. Please obtain the Police check by going to www.police.vic.gov.au

Hospitals are now commencing a policy whereby all Agency staff MUST have a WWCC with them at all times whilst on duty regardless of whether they are working with children.
